Implement approval for TA training

• Similar to Raise Request Approval Page (Access: Coordinator)
  • Maybe an "unverified" state if not approved, verified once approved?
• Once set students shouldn't be able to change it anymore
• Email once a week to coordinators about awaiting approvals
